You'll find copies of each page of our survey instrument (we do show current
data as well) at my ftp site.
We offer no incentives yet achieve a 45% response rate. I am convinced that
showing folk what we have already tends to raise the response rate as they
are more apt to send in corrections if they see anything wrong.

Hi everyone,
Good morning and please excuse the cross-posting.
I'm looking for copies of surveys. I'm particularly interested in seeing
those where existing information on the database is printed on the survey
instrument and then all the alum has to do is make the necessary changes.
Also, I'd like to know what incentive or token of appreciation you give to
your alums to prod them to complete the form and send it back to you.
